You must fill out all applicable sections of the form. Note that if you will require any type of auxiliary marking (colored leg bands, transmitters, etc.), the section of the form dealing with that does not ask for all information that we need. Please attach a page summarizing the details for any auxiliary marking requests, being sure to cover all of the categories described at: http://www.pwrc.usgs.gov/BBL/manual/aarequs.htm. If you do not include all of these details, your application will be seriously delayed.
