Applying for a New Master Permit:
You must fill out all applicable sections of the form. Note: that if you will require any type of auxiliary marking (colored leg bands, transmitters, etc.), the section of the form dealing Auxiliary information does not ask for all of the required information that we need. Please attach a page summarizing the details for any auxiliary marking requests, being sure to cover all of the categories described at Auxiliary Information (see Revised Authorization.)
If you do not include all of these details, your application will be seriously delayed.
Please also include the e-mail address of your references and their permit number if applicable.
